Copy of the Birth Certificate
(From the registrar of birth or equivalent authority. Name of the child must be endorsed in the Birth Certificate duly countersigned by Registrar of Birth)

Copy of Report Card of previous year
(Needed for admission to class 1 onwards)

Passport size Photographs of the child
(Colored/black and white, quantity as specified by school)

Passport size Photographs of both the parents
(Colored/black and white, quantity as specified by school)

Proof of residence
(as mentioned by school, both the original and its photocopy)

Commonly accepted residence proof (any one):
• Passport
• Registered Lease Deed
• Paid Electricity Bill
• Aadhaar Card
• Paid MTNL Telephone Bill
• Electoral Card
• Drivers License
• In case of rented accommodation, an affidavit on stamp paper should be submitted along with any valid photo identity for eg. Driving License, Passport, Voter ID etc

In case of sibling applicants, the identity card/fee receipt of sibling.

In case of child with special needs a certificate of disability (in original) from a government hospital.

In case of single parent:
• Proof of single parent status
• Legal proof of custody of child to be attached
• In case of death of spouse, death certificate to be attached

Immunization / Vaccination records of the child
Kindly note – Formal Schools may have a specified format of Medical Form that needs to be filled and signed by the relevant authority. Medical Form is important for completion of Admission procedure.

In case of SC/ STC/ OBC Category, a certificate from the office of the Sub-Divisional Magistrate
